Introduction. Teaching work exposes educators to multiple demands that may affect their well-being. Therefore, it is essential to identify and monitor teachers’ mental health using valid and reliable instruments. The aim of this study was to evaluate the psychometric properties of the 12-item General Health Questionnaire (GHQ-12) in teachers from public educational institutions in Medellín, and to examine measurement invariance while also exploring, in a complementary way, the structural relationship between its dimensions by sex.
Method. An instrumental, cross-sectional study was conducted with 981 teachers from 150 public institutions (70.1% women, 29.9% men; mean age = 49.3 years). The GHQ-12 was administered using a Likert-type format. The sample was divided into two subsamples: one for exploratory factor analysis (EFA, n = 390) and another for confirmatory factor analysis and structural models (CFA/SEM, n = 591). The analytical sequence included EFA, CFA, a simple structural equation model, configural, metric, and scalar invariance analyses by sex, and a multigroup structural equation model (multigroup SEM). The WLSMV estimator was used in all confirmatory analyses.
Results. The EFA yielded a two-factor structure: Psychological Distress and Social Dysfunction, which together explained 54.0% of the variance. The CFA estimated with WLSMV supported this structure, with satisfactory incremental fit indices and SRMR, although the scaled RMSEA was above the conventional threshold. In the simple SEM, the relationship between factors was positive and strong (β = .663; R² = .439). Scalar invariance between men and women was supported, with minimal changes across nested models (ΔCFI ≤ .003; |ΔRMSEA| ≤ .006). In the multigroup SEM, structural coefficients were positive and high in both groups: men, β = .603 (R² = .364); women, β = .702 (R² = .492). Although the coefficient was numerically higher among women, confidence intervals overlapped and the trajectory-difference test was not statistically significant.
Discussion or Conclusion. The GHQ-12 showed adequate psychometric properties in this sample of teachers. The two-dimensional structure was empirically supported, although its interpretation should consider the potential effect of differential item wording. The relationship between Psychological Distress and Social Dysfunction was strong in both sexes, with no sufficient statistical evidence to conclude that this relationship differs between men and women.
