Clarifying narcissistic organizations involves conceptualizing the opposition/relationship between ego and object. What is the clinical correlate of a metapsychology of the ego and the superego, and what is the metapsychological correlate of a clinical approach to narcissism? The literature on “narcissistic pathologies” reveals a protean array. Thus, I found four modalities: First modality: identity is at stake: schizophrenia, paranoia, and borderline disorders.Second modality: what counts is the value of the ego. Here I situate depression in all its stages.Third modality: the distinction between real and fantasized-thought object is impaired.Fourth modality: Certain ego functions have not been established or have been lost due to excessive suffering.
