Antecedentes: el tratamiento del trastorno cognitivo-comunicativo (TCC) ha demostrado ser una adecuada aproximación para favorecer el bienestar social de personas con traumatismo encefalocraneal (TEC). Objetivo: describir los tipos de tratamiento e ingredientes terapéuticos y su efectividad en el tratamiento directo del TCC en personas con TEC. Metodología: se realiza una revisión sistemática, siguiendo los lineamientos PRISMA, junto a la valoración de la efectividad de cada tipo de tratamiento e ingrediente terapéutico. Resultados: 22 artículos cumplieron con los criterios de inclusión. Tres intervenciones (rehabilitación de habilidades de percepción-regulación emocional y cognición social, habilidades pragmáticas, y conversación/interacción) alcanzan una recomendación A. Todos los ingredientes terapéuticos (educación, metas funcionales, aproximación progresiva, modelado, práctica de habilidades, retroalimentación, simulación, práctica entre pares, ajuste emocional) cuentan con evidencia suficiente para una recomendación A. Conclusiones: el tratamiento directo de la comunicación es propuesto como un abordaje eficaz para personas con traumatismo craneoencefálico. Se discute la necesidad de desarrollar un modelo genérico transversal a los diferentes tipos de tratamiento del TCC, basado en ingredientes terapéuticos.
Background: the treatment of cognitive-communication disorder (CCD) has proven an adequate approach to the social well-being of people with traumatic brain injury (TBI). Objective: describe the types of treatment and therapeutic ingredients — and their effectiveness — available for the direct treatment of CCD in people with TBI. Method: a systematic review following PRISMA guidelines was carried out. The effectiveness of each type of treatment and the therapeutic ingredients available were assessed. Results: 22 articles met inclusion criteria. Three interventions (rehabilitation of emotional perception-regulation and social cognitive skills, pragmatic skills and conversation/interaction) achieved recommendation A. All therapeutic ingredients (education, functional goals, progressive approach, modelling, skills practice, feedback, simulation, practice with peers, emotional adjustment) had sufficient evidence for recommendation level A. Conclusions: direct communication treatment is an effective approach for people with brain injury and should be included as a practice standard. The need to develop a generic model to treat CCD that combines therapeutic ingredients from diverse interventions is discussed.
