Case formulation occupies a central place in child psychotherapy, as it makes it possible to integrate heterogeneous clinical information and guide therapeutic decisions tailored to the child’s singularity. Within this framework, attachment theory offers a particularly relevant perspective for understanding the organization of childhood distress, affect regulation patterns, protective strategies in the face of danger, and the impact of the early relational context. The aim of the present study was to analyze the contributions of attachment assessments to case formulation in children, considering their clinical usefulness, main strengths and limitations, as well as their potential for therapeutic monitoring. To this end, a systematic literature review was conducted in accordance with the PRISMA statement (Page et al., 2021). The search was carried out in PubMed, Dialnet, and Redalyc, and included empirical articles published since 2000 in English and Spanish. A total of 6,409 records were identified, of which 21 empirical studies were ultimately included. The results show that attachment assessments contribute to case formulation through different yet complementary levels of clinical understanding, including caregiver-child interaction, the child’s representational world, protective strategies in the face of danger, trauma, and the parental and family context. Overall, the review suggests that these assessments may constitute clinically valuable tools for developing richer, more individualized, and more useful case formulations in child psychotherapy.
